UCAN Nurse Home Visiting Programs
Through UCAN’s nurse home visiting programs, eligible Douglas County pregnant women, families with young children and families with children with special needs receive comprehensive support services from nurses. Programs offered through Nurse Home Visiting include Maternity Case Management, Babies First and CACOON. These are referral based programs.

Maternity Case Management
Maternity Case Management’s goal is to lower risks for women and their babies, making sure they obtain prenatal care. Through this program, our nurses help pregnant women who have life situations that could lead to problems with their pregnancies or childbirth. Our nurses conduct prenatal and postpartum (after the baby is born) home visits. They evaluate safety issues, nutrition status, and emotional needs, providing education, counseling, relationship support and information and referral as needed. Our nurses help mothers set goals for making healthy lifestyle choices and fostering personal growth. They also identify pregnancy problems or illnesses which mothers may have that may require immediate referral to other healthcare providers.
The goal of Babies First! is to help families make sure that babies are healthy as they grow and learn. UCAN’s Babies First Program nurses offer a number of services to families with children ages 0-5, including:
The goal of CACOON is to provide effective and efficient use of the health care system and community supports. CACOON focuses on community-based care coordination for children with special health care needs. UCAN’s nurses serve children from birth through 21 who have (or who are at risk for) a chronic physical, developmental, behavioral, or emotional condition. Children served through this program require types or amounts of health and related services that are not normally required by children. CACOON program services include:
