How are HRSNs Being Addressed?
Oregon, through local organizations like Umpqua Health Alliance, AllCare and Trillium, is offering funds to help address these needs. HRSN services are offered to those who are on Medicaid/OHP. UCAN is working to provide HRSN services locally.
Services are limited to those who have certain health, living and money issues, and resources are limited. However, this program has allowed UCAN to reach and support many members of our community who are dealing with issues harming their health.
In Douglas County, we can help you complete an application to see if you are eligible for an HRSN service. If you want to find out if you qualify, give us a call at 1-800-301-8226 ext. 2953. You can also contact us at connect@ucancap.org. We will do a pre-screen to see if you may be eligible. If the pre-screen is successful, we will then meet with you to help you complete a full application for HRSN services. You can also ask other local agencies to send us a referral so we can connect with you.
