Health Related Social Needs

What are Health Related Social Needs?

There are many ways you stay healthy. Regular doctor visits are important. But many other things are important to your health, including:

  • Eating healthy foods.
  • Staying housed.
  • Keeping your power on.
  • Breathing fresh air.

When you have a barrier to any of these, you have a health related social need.

How are HRSNs Being Addressed?

Oregon, through local organizations like Umpqua Health Alliance, AllCare and Trillium, is offering funds to help address these needs. HRSN services are offered to those who are on Medicaid/OHP. UCAN is working to provide HRSN services locally.

What Help is Available?

If it looks like you qualify, we can:

  • Connect you with CCO staff who help you keep your Medicaid/OHP coverage
  • Support your access to other benefits, like SNAP, WIC, TANF, Social Security, etc.
  • Access documents that you need to apply for benefits, such as ID Cards, Social Security Cards, and Birth Certificates
  • Connect you with partners to meet added needs, such as legal services, employment or educational supports, etc.

If you do qualify, we are able to provide direct HRSN services to you in Douglas County. These services include:

  • Help paying rent and utilities
  • Tenancy support to maintain your housing
  • Changes to your home, including ramps, grab bars and pull-down drawers
  • Home remedies, including pest control and deep cleaning
  • Help to keep your home cooler/warmer, the air in your home cleaner, and your medical supplies stored safely

Services are limited to those who have certain health, living and money issues, and resources are limited. However, this program has allowed UCAN to reach and support many members of our community who are dealing with issues harming their health.

How Do I Apply?

In Douglas County, we can help you complete an application to see if you are eligible for an HRSN service. If you want to find out if you qualify, give us a call at 1-800-301-8226 ext. 2953. You can also contact us at connect@ucancap.org. We will do a pre-screen to see if you may be eligible. If the pre-screen is successful, we will then meet with you to help you complete a full application for HRSN services. You can also ask other local agencies to send us a referral so we can connect with you.
