UCAN Care Team

Providing Health-Related Social Needs services to our community.

Through Oregon’s Health-Related Social Needs (HRSN) Initiative, OHP members can get help with more than just medical care.

Thanks to a partnership between local Community Care Organizations (Umpqua Health Alliance, AllCare, and Trillium) and community partners, qualified members may receive:

  • Housing Support
  • Nutrition Services
  • Climate-Related Items (like air conditioners or heaters)

These services are available to eligible members going through life changes.

You may qualify for the UCAN Care Team if you’re an OHP member going through certain life changes, like:

How We Can Help You

If you qualify, you may get help with:

  • Getting Assessed for Services (We’ll work with you to gather the info needed – at your pace)
  • Keeping your OHP Coverage (Help with enrolling, re-enrolling, or staying enrolled in Medicaid)
  • Accessing Benefits (Support with applying for programs like: SNAP, WIC, TANF, Social Security & Disability, Veterans Benefits, Housing programs)
  • Getting Important Documents (Help getting ID, Social Security cards, birth certificates, and more)
  • Meeting Basic Needs (We’ll help connect with medical, legal, peer, or educational support)
  • Application Help (Assistance filling out forms)
